How Clinicians Can Partner with Communities to Provide Primary Care to all Mainers, and to Everyone in the US, in Every Neighborhood and Community

Learning Objectives:

After participating in this session, learners will be able to:

  1. Analyze the systemic and economic factors that contribute to gaps in access to primary care in Maine and similar communities across the U.S.
  2. Describe successful models of community-clinician partnerships that improve primary care delivery and equity in underserved populations.
  3. Identify actionable strategies to engage local organizations, stakeholders, and non-traditional partners in the provision of patient-centered care.
  4. Develop a framework for applying community-based solutions to enhance primary care access and outcomes within their own practice or region.

 

This session is accredited for a maximum of 1.0 AOA Category 1-A credits. Credits commensurate with participation in the session will be awarded.

Michael Fine, M.D.

Dr. Michael Fine is an award-winning author, community organizer, family physician, and public health guy.  Dr. Fine is the President and Board Chair Member of Primary Care for All Americans and the chief health strategist for the city of Central Falls, RI.  He was Director of the Rhode Island Department of Health , 2011-2015.  He is currently the Board Vice Chair and Co-Founder of the Scituate Health Alliance and is on the Board of the Lown Institute.
