Learning Objectives:

After completing this activity, learners will be able to:

• Discuss with patients the health risks associated with cardiometabolic risk factors, including the benefits of early intervention
• Outline to patients the available therapeutic approaches which may positively impact their risk for development of cardiometabolic disease
• Commit to utilizing available patient-centered strategies for risk assessment and multidisciplinary care for patients who may be at risk for cardiometabolic disorders such as diabetes, obesity, CVD, and C

  This session is accredited for a maximum of 1.0 AOA Category 1-A credits. Credits commensurate with participation in the session will be awarded.
