Description

When patients experience a trauma it is often a challenge to obtain previous medical history. At the time of discharge there are also challenges related to conveying pertinent information to the primary care providers who will reassume care of these patients. Often times this relates to anticoagulants and risk benefit analysis of continuing these medications. This is an attempt to identify and start to bridge the gaps in patient continuity.

Following this presentation, participants should be able to do the following:
1. Understand and be able to describe challenges faced by trauma services related to past history.
2. Understand and be able to describe challenges to bridging changes to care plan from trauma services to primary care.
3. Be able to discuss and apply the risks / benefits of anticoagulation's amongst the elderly (post trauma).

This activity has been planned and implemented in accordance with the accreditation requirements and policies of the American Osteopathic Association (AOA). The Tennessee Osteopathic Medical Association (TOMA) is accredited by the American Osteopathic Association (AOA) to provide osteopathic continuing medical education for physicians.
